You may request a specific doctor. If you have no preference, please explain the nature of your problem. Each of our doctors has special interests or orthopedic subspecialties. Your visit is important to us and we want the best match for your needs. Subsequent appointments are made with the same doctor to provide the best possible continuity of care.
In certain cases, you may already have X-rays or other medical reports that relate to your appointment. Please bring these X-ray films or other reports to your visit.
Most HMO's and managed care plans require a referral before we can see you. Check with your insurance and primary care doctor if you are unsure about your referral and what will be covered. Insurances with unmet deductible >$1,000 will be required to follow our Self-Pay guidelines. Please see our Patient Privacy Notice for full details.
